Challenged Envelope Statement
If you did not sign the return envelope or Oath of Voter form or if your signature on the envelope does not match the signature in your voter registration record your ballot WILL NOT be counted unless you provide a valid signature. You can provide that signature by completing the form below:

If you are not able to sign the envelope on your own, you can make a “mark” and have it witnessed by a person over the age of 18 for this election. Your witness’s name and signature should appear next to the mark and be noted as a witness. For future elections, you can re-register to vote with your mark on file as your legal signature. If you are not able or willing to make a mark, you may re-register using a signature stamp.
Your signature on your Challenged Envelope Statement must compare to your voter record. If you last registered to vote online or visited the California Department of Motor Vehicles (DMV), the signature in your voter record is likely the one found on your California ID Card or Driver’s License and therefore the signature that will be compared to the signature on your statement.
