Documentary Transfer Tax Affidavit (DTTA)
Effective January, 2011, the Documentary Transfer Tax Affidavit (DTTA), along with any applicable supporting documentation, must be filed with any conveyance document to help explain the nature of the transaction and the validity of the Documentary Transfer Tax exemption, if claimed.
Declaration of Translator
To obtain the translation certification document, the customer must present the required documents to the County Clerk.
Financial Disclosure Forms/Conflict of Interest
The Political Reform Act (Gov. Code sections 81000- 91015) requires most state and local government officials and employees to publicly disclose their personal assets and income.
Military Discharge DD-214
In accordance with Government Code Section 6107, the requestor must be authorized to obtain a certified copy of a military discharge form DD-214.
Preliminary Change of Ownership Report
The PCOR must be completed, signed and filed with any conveyance document except for Easements, Trustee’s Deeds Upon Sale, Deeds in Lieu of Foreclosure and Affidavits of Death (when the decedent is a beneficiary under a Deed of Trust).
Receipt of Returned Notary Journals
Please be sure to complete the following information:
- Notary name
- Notary address and phone number
- Notary commission number
- Expiration date of commission
- Indicate the number of journals surrendered
