Companies mining for sand, gravel, stone, limestone, clay and/or similar products should use this form to file with the Assessor on or before April 1.

Report on a calendar year basis. If this report is prepared prior to January 1, any change in real property between the date the report is prepared and 

January 1 must be reported to the Assessor on a supplemental report.
